Refer a Patient for In-Home Outpatient Physical Therapy

Therapy in Motion fills a specific gap: patients who need outpatient physical therapy and would benefit from it, but for whom getting to a clinic is the reason it does not happen.

Where we fit in the care continuum

The distinction that matters most for referral decisions is this: this is outpatient physical therapy delivered in the home, not home health. Patients do not need to meet homebound criteria. There is no requirement for a qualifying inpatient stay, and the episode is not administered through an agency coordinating multiple disciplines.

In practice, that means you can refer a patient here in the same circumstances you would refer to an outpatient clinic — the difference is that transportation, driving restrictions, fatigue and caregiver scheduling stop being barriers to attendance and completion.

Common referral scenarios

  • Post-operative orthopedic patients under driving restrictions, particularly in the early weeks after joint replacement or repair
  • Patients discharged home after a hospital stay who need outpatient-level therapy but are not homebound
  • Older adults with balance decline or fall concerns where the home environment itself is clinically relevant
  • Patients with Parkinson's disease or other neurological conditions for whom clinic attendance is inconsistent
  • Patients needing assistive device training who will use the device in a specific home layout
  • Patients living in Rathdrum, Hayden or outlying areas where travel distance affects adherence
  • Patients who have previously started and discontinued clinic physical therapy for logistical reasons

How to refer

Send the referral by fax, or call the practice directly to discuss a patient before referring. We will contact the patient to schedule and verify benefits, and we will let you know if a referred patient turns out not to be an appropriate fit for in-home care.

Evaluation and treatment across the full range of services we provide, including neurological physical therapy, fall prevention and balance, post-surgical rehabilitation, musculoskeletal conditions, functional dry needling, mobility and assistive device training, and caregiver education.

Questions from referring providers

Is this home health?
No. Therapy in Motion provides outpatient physical therapy delivered in the patient's home. Patients do not need to meet homebound criteria, there is no agency plan-of-care oversight structure, and there is no requirement that the referral follow a qualifying hospital stay. Functionally it is an outpatient PT episode, with the visit location changed.
Which patients are a good fit for this model?
Patients who would benefit from outpatient physical therapy but face a barrier to attending a clinic — transportation, driving restrictions after surgery, fatigue, anxiety about falls, caregiver scheduling, or distance from Coeur d'Alene. It also suits patients whose goals are inherently environmental, such as stair negotiation, transfers, bathroom safety or assistive device use in their actual home layout.
Which patients are not a good fit?
Patients who require services outside the scope of what can be appropriately provided in a home setting, or who need equipment or monitoring not available in that environment. If a referred patient is not an appropriate fit, we will tell you promptly rather than proceeding, and will communicate what setting we think would serve them better.
Do you accept Medicare and Medicaid?
We work with Idaho Medicare Part B and Idaho Medicaid, along with several commercial plans. Coverage and benefits vary by individual plan, and we verify benefits with the patient before the initial visit.
What is the coverage area?
Kootenai County, including Coeur d'Alene, Post Falls, Hayden and Rathdrum. For patients slightly outside those communities, call and we will give you a direct answer about whether we can reach them.
Who provides the treatment?
Dr. Natasha Lee, DPT and Janice Wright, PTA. The practice is two clinicians, so a referred patient sees the same provider throughout the episode of care rather than rotating staff.
